We are seeking a dedicated Hospital Social Worker to join our innovative Integrated Discharge Team at the Northern Care Alliance NHS Foundation Trust. This role is pivotal in facilitating safe and timely discharges from the hospital, using a strengths-based approach to support patients and their families.


Day To Day Of The Role


  • Work within the Integrated Discharge Team to support wards in facilitating patient discharges.
  • Apply a HOME FIRST Approach to support patients in the wards and community settings.
  • Engage with multi-disciplinary teams to provide personalised support, promoting independence and choice.
  • Undertake statutory Social Work functions in line with the Care Act and the Mental Capacity Act.
  • Support people with complex needs, ensuring they have maximum choice and control.
  • Work on a 7-day service rota basis, including weekends (8:45 am - 5:00 pm).


Required Skills & Qualifications


  • Social Work England registration.
  • Eligible to work in the UK.
  • Hold a full UK driving licence.
  • Minimum of 5 years post-qualifying experience.
  • Practice educator training or progression to Level 3 is ideal.
  • Experience in supporting and mentoring ASYE social workers and students on placement.
  • In-depth understanding of the legal frameworks relevant to Adult Social Work.
  • Excellent listening skills, highly organised, and a team player.
  • Passionate about working with adults in a strengths-based way.
