This role reports to the Service Manager The role provides a service or support to:

Clients of the service and the target population of the service

The post holder will contribute to the delivery of the Live Life Better Derbyshire service. This will involve supporting clients to identify and address their holistic wellbeing needs including the direct provision of weight management, physical activity and smoking cessation support. The service is aimed at individuals aged 16 and above (stop smoking aged 12 and above), and families with children under 16.

Ensure client confidentiality and the safeguarding of vulnerable children and adults.

Provide support to access specialist support services to address an individuals priority needs e.g. debt advice/housing advice and refer clients with more complex needs into the appropriate specialist services.

Build and sustain effective relationships with partner agencies in order to effectively signpost clients to the appropriate services.

Delivery of training to professionals.

Offer a flexible service based on local need, this may include travel across Derbyshire and some evening/weekend working.

Identify and engage effectively with members of the community from all backgrounds and abilities, listening and interpreting complex health and wellbeing information into understandable terms.

Work independently in a locality to support clients to undertake a holistic wellbeing assessment to identify priority areas forchange and use specialist behaviour change techniques to address identified need.

Work independently to offer intervention support for two lifestyle areas (weight management, smoking cessation, physical activity), offering brief intervention support in the third area.

Organisation, delivery and management of group and 1:1 stop smoking and/or weight management/physical activity support sessions in variety of settings, including the provision of specialist behavioural support. Direct supply and dispensing of pharmacotherapy will be required for stop smoking services only.

Assist clients to change their lifestyles and to be able to encourage them in choices and actions that are acceptable and achievable to them while being aware of barriers such as affordability, accessibility and cultural considerations whilst taking self-disclosed medical history and co-morbidities into account.

Maintain accurate information systems of client records and activities, ensuring information is fit for purpose for reports, audits and contributes to the evidence base for the health improvement service.
