We are passionate about rehab. Our aim is to improve the overall well-being and functional abilities of older adults, helping them maintain their independence and enhance their quality of life as they age.

When an elderly person is discharged from hospital following surgery, an illness, injury, or serious medical event such as a stroke, it’s vital to ensure that they have the best follow up rehab services in place, to aid their recovery and to give them the best possible quality of life. In many cases, a stay in an inpatient rehab facility will give them the best chance of regaining their health, independence, and quality of life, in a safe and properly equipped environment, until they are able to be safely discharged.

  • Customer Care for Patients and/or Service users
  • To adhere to Code of Conduct/Professional Standards of your Profession e.g. NMC or HPC
  • As a clinician to embrace and implement the vision and values of HHFT.
  • To be responsible for contributing to the creation and maintenance of a clinical environment in

which care and compassion is consistently demonstrated, ensuring patient centred care, privacy

and dignity is practiced at all times.

  • To adhere to the trust and local Child Protection and Safeguarding Policy at all times
  • To support the team to ensure and maintaining a high quality seven day, twenty-four hour service for all aspects of their work and the staff in their area /department.
  • To maintain, develop and record your own continuing professional development, including

booking and attending all statutory, mandatory and Trust or speciality specific training

  • Clinical nursing care
  • Medication management
  • Swallow management
  • Provision of medications
  • Discharge planning
  • Wound care
  • Nutritional management
