Job summary

Thepost holder will work as part of the Health Centre Team in collaboration withthe general practitioners and develop an effective, quality driven practicenurse service. The post holder will promote integrated nursing practice withinthe wider nursing team underpinned by the development of evidence-based practice guidelines, forshared delivery of care to the whole practice population. Further, to ensure clinical effectiveness bymonitoring of key quality markers relating to guidelines

Attendand participate in practice meetings, including the Multi-Disciplinary Health Care Team Meetings as required.

Tomaintain up-to-date personal professional development and awareness throughreading and attendance on courses as agreed with the lead GP.

Tomaintain effective liaison with other agencies and staff concerned with patientcare and with all other disciplines within the practice with appropriate regardto confidentiality.

Toensure that the key requirements of the current Health and Safety at Worklegislation are understood, in that it is the personal responsibility of everyemployee to work and behave in the manner which protects the safety of theircolleagues and the patients and to ensure their work place is maintained in asafe, clean and tidy condition at all times.
