Job Description
We are seeking a dedicated and experienced Qualified Social Worker to join our Hospital Team in Wolverhampton. This full-time locum role offers a fantastic opportunity to make a significant impact on the lives of individuals during critical times of their health journeys.
Key Responsibilities
Conduct comprehensive assessments of individuals' needs and develop care plans to facilitate safe and timely hospital discharges.
Work collaboratively with healthcare professionals, patients, and their families to ensure a coordinated approach to care.
Provide expert advice and support to patients, helping them to access community resources and services.
Act as a liaison between the hospital and community services to ensure continuity of care.
Maintain accurate and detailed case records and documentation.
Participate in multi-disciplinary team meetings and contribute to case conferences and discharge planning.
Requirements
Qualified Social Worker registered with Social Work England.
Experience in hospital social work or a related field within Adult Social Care.
Strong understanding of relevant legislation, policies, and best practices in hospital social work.
Excellent communication, negotiation, and interpersonal skills.
Ability to work effectively under pressure and manage competing priorities.
Commitment to continuous professional development and learning.
Valid UK driving licence and access to a car (exceptions may apply under the Equality Act).
