Our diverse, talented and caring team include frailty consultants, a consultant advanced clinical practitioner (ACP), GPs, consultants-in-training and trainee ACPs from a broad professional background. Together we support a wide range of activities including:Home Treatment ServiceResearch tells us that people prefer to receive their care closer to home and that most people wish to stay at home for as long aspossible. Consequently a dynamic new service was set up at pace during the pandemic to address this need locally.The seven-day service provides expert frailty advice and input throughout east Kent. Receiving referrals from any clinician, our aim is toprovide alternatives to hospital admission for those with a frailty crisis. Near patient testing and hospital-at-home capabilities haveallowed us to deliver robust alternatives to ED attendance, including the active treatment of over 200 patients with covid and hypoxia.

Medical and Dental colleagues are expected to:o Undertake the clinical and administrative duties outlined in the job plan - to be agreed on appointment and reviewed regularly.o Maintain CPD in order to maintain professional registration and achieve revalidation.o Work closely with colleagues in other organisations in order that services are provided seamlessly across health and social careboundaries.o Undertake annual appraisal, developing a personal development plan that includes clinical competencies reflecting the health needsof the local population and relates to Trust strategy.o Be actively involved in the teaching, education, supervision and mentoring of clinical staff, including nurses and junior doctors.o Promote a non-judgemental interest in patients and to contribute to a professional team approach to the delivery of high-qualityservices.o Practice with the professional values and behaviours of all doctors as set out in the GMC documents, Good Medical Practice andGeneric Professional Capabilities Framework.o Maintain an awareness of legal responsibilities relevant to the role, such as mental capacity, deprivation of liberty, data protectionand equality and diversity.

Additional Working Pattern: Includes 1 in 5 weekends, and occasional 6-8pm shift
