- Support the PCCM with the development, coordination and administrative duties required to deliver the PCC initiatives electronic Holistic Needs Assessments, care plans and treatment summaries, health and wellbeing events/clinics and other supportive initiatives.
- Act as a point of contact for internal and external enquiries regarding the Personalised care and Stratified Follow-up.
- Support the PCCM to coordinate promotional activities in the local community.
- Support the PCCM to provide education and information sessions to health care professionals within the Trust and community where appropriate.
- Support the PCCM to take appropriate actions to develop/refine the service according to need.
- Assist with all administrative duties and data management in close collaboration with the PCCM
- Attend meetings as necessary to record and produce accurate minutes for distribution in addition to disseminating relevant information.
- Assist the PCCM to organise local Health and Wellbeing events (eventually taking over responsibility of coordinating these events)
- Ensure up to date knowledge of current legislation, national and local regulatory requirements relating to accurate information provision.
- To effectively communicate with other health care professionals, volunteers and service users.
- Foster excellent working relationships with internal and external contacts at all levels.
- Make judgements on when to escalate concerns to CNS teams or PCCM (particularly when undertaking patient assessments) and knowing when to refer to other parties (e.g. Macmillan Cancer Information and Support Service)
- Undertake any other duties that may be required from time to time as are consistent with the responsibilities of the grade and the needs of the service.
Are you someone with excellent communication skills who would like to support people with cancer? We have several opportunities here at the North Middlesex Hospital working aspart of the Clinical Nurse Specialist Cancerteams to help assess and offer support using a Holistic Needs Assessment to our cancer patients during their cancer pathway and beyond.
The post holder will work closely with the Personalised Cancer Care Manager and Clinical Nurse Specialists to develop care plans and health and well-being programmes, ultimately improving the patient experience.
The Cancer Support Worker will be required to initiate and maintain effective working relationships with a wide range of staff across the Trust and North Central London including colleagues, service users, carers and other stakeholders.
The post holder will be required to work on tasks that have implications for the successful implementation of the Personalised Cancer Care & Stratified Follow-up at North Middlesex University Hospital NHS Trust.
Main duties of the role are as below and the list is not exhaustive:

- to assist in the implementation of the personalised care agenda
- to collect and prepare data on service activity and use feedback for quarterly reports for PCCM, NMUH Cancer Board, North Central London (NCL), Alliance Dashboard and audit meetings as required
- support the PCCM to deliver national Survivorship initiatives
