To work under the direction of the Patient Support Supervisor to deliver the stratified pathways for post cancer treatment.
An exciting opportunity has arisen for an enthusiastic and motivated nurse to join our Open Access Follow Up (OAFU) service. OAFU is a remote monitoring service for patients after completion of cancer treatment. This stratified follow up supports patients to self-manage, reducing hospital attendances and providing a better patient experience, while still safely providing regular monitoring for cancer recurrence. This service is currently primarily offered to Breast, Colorectal and rostate patients.

We are looking for a nurse to provide clinical support and expertise to the team of cancer support workers. You will primarily focus on supporting patients to transition from regular clinical follow up to remote monitoring. Providing holistic needs assessments for patients while supporting self management and delivering health & wellbeing interventions. You will support the admin team with care co- ordination, complex care planning including referral triage & discharge and support with helpline calls for patients entering the remote monitoring pathway.

The ideal candidate will have advanced communication skills, experience of holistically supporting and assessing patients, and established administrative skills; including excellent ICT skills with a good understanding of Excel. Previous experience working with cancer patients would be beneficial but not essential.

This role is a rare opportunity for a band 5 nurse, offering the chance to provide high quality support and care in an autonomous nursing role, that can influence the cancer patient experience.

  • To provide high quality, effective, individualised care to all patients that is compassionate and caring.
  • To support patients health and wellbeing after cancer treatment, with a particular focus on promoting self-management
  • To work under the direction of the Patient Support Supervisor to deliver the stratified pathways for post cancer treatment
  • To assess holistic care needs and develop, implement and evaluate them with necessary supervision, signposting or referring patients to the relevant support
  • To effectively liaise and communicate with primary care services to ensure patients are suitably supported on a stratified follow up pathway
  • To keep up to date with skills and knowledge
