You will support practice staff with prescription and medication queries. Help support the repeat prescription system, deal with acute prescription requests, and medicines reconciliation, providing expertise in clinical medicines advice while addressing both public and social care needs of patients.
You will support with prescribing practices across the PCN, providing clinical leadership on medicines optimisation and quality improvement.
East Kennet PCN has a total population size of approximately 38,000 patients consisting of 4 practices who provide a collaborative way of working across the PCN. The PCN provides excellent support to all teams and works in collaboration with The Pharmacist Network team. You will be supported by the practice and the current pharmacist provision in-house as well as Senior Pharmacist support from The Pharmacist Network. As a pharmacist, you will be aligned to 1 or 2 practices where you are able to make a real impact within the practice and develop your experience and expertise too.
