Discussion with patients and relatives about what support is needed in the discharge process.
Discussion with multidisciplinary team members including social services, the discharge management team and district nurses.
Discussion with ward physiotherapists and occupational therapist.
Liasing with discharge lounge to arrange transport home.
We are recruiting to the post of Discharge Coordinator on an Endocrine ward. The post holder will be responsible for the arranging of safe discharges from the ward. We are a friendly team and are always keen to welcome new members to the ward.
The role of the successful candidate will be to arrange discharges of the patients. This will entail communication with patients, relatives and the multidisciplinary team to implement the services needed for safe discharge of our patients.
