Job summary

This is a new role, to work with thePrimary Care Network Pharmacist and Clinical Director, coordinating the delivery of pharmacyservices across the network, in line with the PCN Direct Enhanced Service (PCNDES) requirements or locally agreed PCN workplans. We expect the role will develop, and the job description is intended to be a guide only.

The PCN consists of 4 practices covering a wide geographical area in East Riding of Yorkshire and although there will be some elements of travel between the 4 practices the majority of this role will be working remotely.

It is also vitally dependent upon the engagement of patients in adapting to healthy lifestyles to help prevent ill-health.

Toundertake work in line with PCN directed priorities.

SupportQuality and Outcome Frameworks, other DES/LES specifications, PCN DES targetsand other locally agreed PCN targets.

Providecoordination and navigation for people and their carers across health and careservices, aligning work to reflect the PCN needs for pharmacy serviceprovision.

Toliaise with PCN Pharmacist on service delivery, agreeing annual targets toreflect PCN DES and/or clinical targets as determined locally and agreed by PCNClinical Director.

Tosupport the design and ongoing implementation of structured medication reviewand care homes review services.

Toliaise with care home staff across the network, and other relevant colleagues,in the coordination of a care homes pharmacist visitation service.

Toencourage and arrange the provision of care for housebound patients, those whoare lost to follow up, and to support the inequalities work of the PCN.

Todevelop and run clinical searches, with support from the PCN pharmacist, toidentify patients who would benefit from pharmacist intervention and/or review.

Liaisewith the care co-ordinators working at other network practices to share anddevelop best practice at PCN level.

Proactivelyidentify and work with a cohort of patients who are high-risk for medicationrelated harm, or have polypharmacy and/or multimorbidity, to ensure they areengaged with primary care and accessing review and follow-up services.

Ensureregular and consistent communication with the PCN pharmacist regarding patientprogress and any complications or guidance.

Liaisewith PCN pharmacist/other relevant clinician to ensure tasks are completed andcare delivered in reasonable timeframes.

Actas the first port of call for caseload patients in relation to their careacross the network practices.

Directlyliaise with multi agencies/care providers to coordinate care for patients.

Co-ordinatereferrals as required by PCN pharmacist across the network.

Carryout searches on EMIS to identify patients for recall and ensure invitations aresent in a timely manner via telephone, text or letter and ensure this is readcoded to patients medical record.

Tosupport patient/carer contact roles and collate patient and carer feedback ontheir experiences of PCN services.
