Job summaryAn exciting opportunity has arisen where we are looking to recruit a Nurse Practitioner to work at our practice as part of our multidisciplinary team consisting of specialist nurses, practice and practitioner nursing, paramedics, pharmacist, gps and hcas, in addition to an admin and finance team and pcn workforce.
The successful applicant will be required to assist medical personnel in the care of practice patients. They will be able to assess, diagnose, plan, implement and evaluate treatment / interventions and care for patients presenting with an undifferentiated diagnosis working both autonomously and as part of a mdt practice.
The nurse practitioner should be competent and confident in managing both new diagnosis and long term conditions and will work at both our Bedford Street Surgery and branch site at Furtzon Medical Centre with the support of our gps.
The practitioner should be able to produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures and deliver care in accordance with guidelines.
A prescriber would be preferred but is not essential. Systm one experience would be preferred but training will be provided.
Main duties of the jobAssist medical personnel in the care of practice patients which include treatment, preventative care, screening and patient education.
Consult with patients to provide assessments, screening treatment services and health education advice and where appropriate refer to other services.
Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate).
Diagnose and manage both acute and chronic conditions, integrating both drug-and nondrug-based treatment methods into a management plan.
Support patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care.
Regular attendance to shared care and learning meetings and an active participant in team meetings.
Be part of the practice vaccination programmes.
