The post holder will be based within the Integrated Mental Health Service (IMHS), within Woodlands Resource Centre, FCH. This is a permanent post. The post holder will support CPN team to provide care to those experiencing mental illness and receiving care from secondary care mental health services.

We are seeking a dynamic and forward thinking candidate, to assist in providing support for CPN team to offer direct clinical input to patients with a range of mental health issues.

You will be responsible for the delivery of direct care to an allocated caseload of clients on an individualised basis under supervision of registered staff.

You will also be responsible for informally assessing patient progress and presentation, reporting to registered nurse/department manager. Experience working in a mental health setting would be advantageous, however not essential.

The post holder would work across the Falkirk Health and Social Care Partnership and will interface across NHS Forth Valley, Housing Services, Substance Misuse Services, SPS and other agencies to provide an accessible and responsive service to meet the mental health needs of individuals, thus helping to support the individual’s engagement and ultimately improve outcomes.

Ability and willingness to travel throughout the area of service delivery is essential for this post.
