Gateshead Inner West PCN are seeking an experienced, self-motivated, hard-working, and patient focused nursing associate to join our team.

The post holder will work closely with our GP's and other primary care professionals within the PCN to proactively identify and manage a caseload of patients within the community, making sure that support and care is made available to them and that their complex needs are addressed.



This role is offered on a full-time basis, for 37.5 hours per week, Monday - Friday.

  • To support the organisation and facilitation of clinical care, in a holistic and proactive manner, to patients registered within our PCN.
  • To maintain a high professional standard of nursing care, in adherence to all PCN protocols, policies and guidelines, as well as to provide nursing assistance to the GP's and other members of the primary healthcare team
  • To demonstrate flexibility, in line with the present atmosphere for change within the health service delivery.
  • To work within your own level of competency, and expanding the role in accordance with PCN requirements and own scope of practice
