H@H has a varied caseload of patients admitted for acute medical care provided in their own homes.
An exciting opportunity has arisen for Band 5 staff nurses within the Hospital @ Home team based at Woodend Hospital. H@H has a varied caseload of patients admitted for acute medical care provided in their own homes. Currently our work is focused within the older adult population, respiratory pathway, out patient antibiotic therapy and looking to expand into delivering acute medicine admission avoidance.

The post holder will work autonomously and also be required to work as part of a multi-disciplinary team alongside and in conjunction with Consultant Geriatricians, GPs, Advanced Clinical Practitioners, nurses, Physiotherapists, Pharmacy staff, Occupational Therapists, Speech and Language Therapists.

We are looking for motivated team members to provide a high standard of patient assessment and care. Preference will be given to applicants who have current hospital or community experience.

Applicants must meet the requirements of the person specification to be considered for these posts. All applicants must be able to drive and have access to a car for work purposes.
