Wehave five partners and four salaried doctors, established paramedics and an experiencednursing team. We are looking to developour team further following the retirement of a long-serving partner.

We are part ofa local PCN, accessing funding for resources to improve the service providedand reduce the burden on doctors. Wehave our own in-house pharmacist and have administrative staff who are trainedin workflow optimisation to deal with clinical paperwork. We work on EMIS web.

Job Description for Allied Healthcare Professional inGeneral Practice:


Title: Advanced Nurse Practitionerin General Practice

The successfulapplicant will provide care for the presenting patient from initial historytaking, clinical assessment, diagnosis, treatment and evaluation of care. This may take place in the surgery, thepatients home or a residential home.They will be encouraged to develop a self-directed role within the team,which may vary in responsibilities from day to day. Telephone assessment, home visits andface-to-face assessments at the surgery will be the mainstay of their workload,however development of care plans and long-term condition management will playan important role too. They willcommunicate and work collaboratively with the general practice team to meet theneeds of patients and support the delivery of a safe and efficient service.

Responsibilities:

Assess,diagnose, plan, implement and evaluate treatment/interventions and care forpatients presenting with an undifferentiated diagnosis

Telephonetriage of patients requesting same day appointments and other urgent queries,prioritising the needs of patients and referring on to other members of thePrimary Healthcare Team as necessary.

Clinicallyexamine and assess patient needs from a physiological and psychologicalperspective, and plan clinical care accordingly

Assess,diagnosis, plan, implement and evaluate interventions/treatments for patientswith complex needs

Proactivelyidentify, diagnose and manage treatment plans for patients who have, and are atrisk of developing, a long-term condition

Diagnoseand manage both acute and chronic conditions, using knowledge of both drug- andnon-drug-based treatment methods into a management plan

If youare a prescriber, you would initiate and review medication appropriate topatient needs and in accordance with evidence-based practice, appropriate protocolsand within scope of practice
