The role of the Practice Development Nurse will be to support the Primary Care Professional Nurse Lead to identify and develop the education and training needs of Primary Care Nurses across the region. In line with strategic frameworks, policies and best practice guidelines training delivered needs to, meet the needs of a diverse patient population group.
The Role required the post holder to:o Promote high standards of care and professional standardso Have expert knowledge and skillso Empower and support staff in their developmento Foster a culture of learning across Primary Care to deliver exceptional care standards.The post holder will act as an advocate and lead for the GPN workforce across the Health board establishing links with Localand National forums and Groups to deliver training programmes. Work alongside education leads to access fundingopportunities for the post registered Nursing workforce, Health care support workers and support facilitation of placementsfor Pre-Registration students in Primary care.Participate in clinical audits and research to identify areas for quality improvement, developing learning packages tofacilitate improvements, acting as a change agent to ensure the best care is delivered.
o Work collaboratively with the DHON for Primary Care, Primary Care Academy and those within multidisciplinary teamsto facilitate high standards of training covering a multitude of competencies and opportunities to increase staffengagement in training to develop their own practice.o Demonstrate expert knowledge and advanced clinical skills relevant to speciality to act as a role model providing highstandards of care underpinned by best practice guidelines and relevant practical experience.o Identify areas of practice within the Primary care where advanced skills may be required and take the lead in ensuringadequate numbers of nursing staff maintain competency in this area for patient safety.o Take the lead in developing education and training packages to ensure recommendations form audits are delivered.o Provide leadership, motivation and vision to staff to aid development and manage change.o Confidently lead group meetings discussing statistics and reports for practice development improvementso Facilitate implementation of clinical risk management principles in line with Primary Care contractual procedureso Work collaboratively with The DHON and Nursing Group Director to address and respond to any incidents, complaintsor investigations arising as a result of poor practice.
