ICASS (Integrated Community Assessment & Support Service) has the overall aim of improving the quality of care and outcomes for older people with the intention of allowing them to remain independent within their own homes or communities. A key aspect of the role is supporting the patient flow through the health system by facilitating early supported discharges from the acute hospitals and also by preventing admission to hospital. This will be done by providing nursing interventions to patients on the caseload, and also by taking on a key worker and generic assessor role within the team.

This post sits within the West Division with the main base being Dunfermline, however work will at times be required across sites.

You should have excellent communication skills, be able to demonstrate sound clinical skills required for the role, decision/ reasoning skills, and good time management/organisational skills. You should be able demonstrate person centered care, have good understanding of Rehabilitation and Multidisciplinary working and have a good understanding of the local community and resources. In order to meet the challenges of this evolving service, you need to have a genuine interest in caring for patients in their own home and a flexible approach to working in the community is essential.

This post involves flexibility to work over a 7 day period.

An orientation programme will be available to support you. You will also be involved in supervision and performance management of support staff. NHS Scotland is reducing their full-time working week from 37.5 to 37 hours per week from 1 April 2024 but with no change in pay. This reduction will also be applied pro rata for part time staff. This advert and any subsequent offer/contract of employment therefore reflects the new working hours. However, as not all service areas will be able to adopt the 37 hour working week immediately from 1 April 2024, you may be required to work up to an additional 30 minutes per week for a temporary period for which you would be paid until the service you are working in changes rosters or working patterns to accommodate the new reduced working week.
