A vacancy has arisen for a Family Partnership Practice Lead (FPPL) working within the Tonbridge and Malling district. The FPPL has a key role in embedding the Family Partnership Model within Kent Health Visiting Services under theguidance of the Family Partnership Programme Manager.
The Family Partnership Model is a strengths-based approach. The evidence-based model supports, empower and enable parents and families to overcome their difficulties, buildstrengths and resilience and fulfil their goals more effectively.Working as part of the local Health Visiting team, the Family Partnership Practice Lead will facilitate the earlyidentification of vulnerable families and support the delivery of the KCHFT Family Partnership Programme by managing asmall caseload of families, supporting them ante-natally and until their infant is a year old.In order to facilitate the delivery of the Family Partnership Model, the post holder will have a lead role in embedding theFamily Partnership Model within district health visiting practice as well as providing leadership and some linemanagement to the district health visiting team.The work with clients is intensive and challenging but provides a real opportunity to make a difference to their lives. Youneed to be a resilient and flexible Health Visitor with excellent communication and team leadership skills.

They will ensure completion of agreed outcomes for delegated Children Protection cases as per agreed multi-agency care plane ensuring regular communication with caseload holder.The Family Partnership Practice Lead will be expected to lead on case discussions in partnership with the District Managerand Senior Health Visiting Practitioner.
