We are Lincolnshire Community Health Services NHS Trust (LCHS), the primary community healthcare provider in Lincolnshire delivering services aimed at supporting people to manage their own health closer to home an reducing the need for people to go into acute hospitals. We are rated 'Outstanding' by the Care Quality Commission and are playing an integral part in shaping future health service provision countywide alongside partner health and social care organisations.
Adult Community Therapy offer holistic therapy assessments in the patients' own home and within care homes, aiming to increase independence and prevent hospital admissions, where possible. We also manage transitional care beds; assessing these patients in a timely manner to facilitate discharge planning and reduce length of stay. Care planning and patient-centred goal planning is a fundamental part of the patient's journey and integration with social care teams, nursing teams, and other stakeholders is essential.
There is a robust support network from the local and countywide Therapy Team and the opportunity to learn from other services and colleagues, making excellent opportunities for qualified practitioners. We have a dedicated team of Clinical Practice Educators who offer support and we provide in-service training and regular supervision which helps identify any learning needs whether internal or external.
You will require a Physiotherapy Degree, and current Health Care Professions Council registration, As this is a community post, it is essential that you have the ability to travel between locations to carry out your work and to attend training and meetings: daily access to transport is therefore required.
