Band 6: £35,392 to £42,618 a year per annum  BANK 
To provide patients and their carers with a specialised therapy plan that promotes the development needed to acquire the physical skills to live as independent a life as possible and reach their maximum potential.

  • To be an active member of the Adult Community Therapy Team in delivering person centred care with particular focus on clinical and rehabilitation interventions, enabling individuals to achieve their optimum physical, psychological, social and emotional wellbeing.
  • To independently assess patients referred into the service / speciality using a specialist assessment to determine clinical diagnosis and treatment indicated and maintain records as an autonomous registered therapist within a variety of clinical settings.
  • To determine clinical diagnosis and most effective specialist physiotherapy management for patients referred into the service based on specialist theoretical knowledge and practical based experience.
  • To manage a clinical caseload of patients with complex and specialised needs to a high professional standard and seek guidance from senior colleagues when necessary
  • To provide patients and their carers with a specialised therapy plan that promotes the development needed to acquire the physical skills to live as independent a life as possible and reach their maximum potential
  • To work as an autonomous registered therapist and be responsible for the overall management of patients referred into the service including triaging referrals, managing patient flow through the therapy pathway and undertaking the daily coordinator role.
  • Ensure that patients are seen within agreed waiting times.
  • To work towards and achieve role specific competencies.
  • To be a key member multi-disciplinary and multi-agency team and in partnership with other services and agencies including Social Services and to ensure that physiotherapy is fully integrated into the patients care. To coordinate team and local multi-disciplinary meetings.
  • To communicate effectively with other health professional and outside agencies where appropriate.
  • To participate in clinical supervision and training.
  • To contribute to the service targets and key performance indicators to reduce length of stay, monitor and assist with patient flow across therapy pathways, preventing unnecessary hospital admissions and effective discharge process by providing timely community based rehabilitation and community care.
  • To maintain high quality standards in all aspects of care delivery, working in partnership with primary care colleagues, other health professionals, patients, carers, statutory, independent and voluntary organisations.
  • To provide training and leadership to student physiotherapists, junior therapists, therapy support staff, peers and other health and social care colleagues.
  • To contribute on local service development activities including audit and research.

Adult Community Therapy offer holistic therapy assessments in the patients' own home and within care homes, aiming to increase independence and prevent hospital admissions, where possible. We also manage transitional care beds; assessing these patients in a timely manner to facilitate discharge planning and reduce length of stay. Care planning and patient-centred goal planning is a fundamental part of the patient's journey and integration with social care teams, nursing teams, and other stakeholders is essential.

There is a robust support network from the local and countywide Therapy Team and the opportunity to learn from other services and colleagues, making excellent opportunities for qualified practitioners. We have a dedicated team of Clinical Practice Educators who offer support and we provide in-service training and regular supervision which helps identify any learning needs whether internal or external.

You will require a Physiotherapy Degree, and current Health Care Professions Council registration, As this is a community post, it is essential that you have the ability to travel between locations to carry out your work and to attend training and meetings: daily access to transport is therefore required.
