We are currently looking for a dynamic Staff Nurse to join our team on The Discharge Lounge at The Royal Shrewsbury Hospital.
Main duties of the job
To be responsible for a safe discharge of our patients from the discharge lounge to their destination.
Making sure discharge letters and medications are with the patient before discharge
Patient centred care, assessing individual patient care needs and developing, implementing and evaluating plans of care for a group of patients in accordance with agreed policies and professional guidelines.
Transferring patients from the wards to the discharge lounge in a timely manner to help with capacity and flow.
Making sure all referrals are made for our patients who need them.
To take charge of the ward for a span of duty, normally working under the supervision of senior staff.
To work flexibly to meet the needs of the service.
