Job Overview


To assist in the provision of nursing care (psychological, spiritual and sociological as well as physical), to patients in hospital as a member of the proactive nursing team. Direct care will be delivered to patients suffering a range of problems including: dementia/confusion, falls/fractures following strokes, heart and respiratory conditions as well as palliative and terminal care. To have the ability to recognise and report any changes in a patient’s condition and needs.


Main duties of the job


Clinical/Professional Duties

  • At the direction of the Registered Nurse, participate fully in the delivery of planned nursing care, using a wide range of clinical skills.
  • Provide reassurance and support to patients, carers/family.


Patient/Client Care

  • Involve patients and carers/relatives in delivery of patient care.
  • Provide escorts for patients and undertake transfer of patients as directed.


Communication Skills

  • Work collaboratively with others in the ward to ensure the patients’ needs are met.


Education and Research

  • Develop own skills and knowledge and contribute to the development of others.  Participate in KSF appraisal review as per the Trust policy. Open Compassionate Empowering
  • Attend mandatory ‘in-service’ training programmes including: moving and handling, fire etc.
  • Fulfil the role of link nurse as required.
  • Attend and participate in ward/team meetings.
  • Participate in patient and public involvement activities.


Policy/Service Development

  • To be aware of and adhere to the Trust’s policies and have knowledge of local policy station.


Freedom To Act

  • Recognise and respond appropriately to urgent and emergency situations.
