We have an exciting opportunity to recruit 1 practitioner to support with patient flow across the acute and community. The opportunity is at Pilgrim Hospital, Boston.
The purpose of the role is to support with triaging within the Transfer of Care hub within the acute hospital with the aim of facilitating discharges for patients from the acute into LCHS services.
The role is an exciting role that requires an individual to be diciplined with time keeping, excited about learning about new ways of working and they should not be phased by fast paced changes that can fall within the working day to support the Acute hosptial.
It is an exciting prospect that would suit someone who would like to be able to work flexibly within our team.
The practitioners based at Pilgrim acute hospital sites will be part of an integrated discharge hub to support in triaging and influence pathway decision making for those medically optimised patients.
It is vital that the successful candidate has the ability to be flexible, work efficiently and bring healthy challenge to the meetings while keeping the focus on patient's wishes and care and the forfront of their decision making,
The role requires daily stakeholder engagement and the successful candidate should have the qualities in making and developing good relationships with the stakeholders.
The key features of the candidate is an autonomous worker who can bring healthy challenge and be the patients advocate when needed.
