Our PCN is a dynamic, friendly network of like-mindedpractices, striving to provide effective, individualised care to over 37,000patients in the East Durham area. The successful candidate will benefit fromthe support of the wider PCN team and have expertise in carrying out long-termcondition reviews (including frailty) to patients within our aligned care homesand in the community for those who are housebound. You will be expected toutilise your skills in clinical assessment and diagnostics to make decisionsaround patient treatment plans.
