Durham Coast Primary Care Network are looking to recruit anEnhanced Nurse to work closely with our practices, to enhance our team anddeliver quality care to a targeted cohort of patients.

Main duties of the job

This is a new role to our PCN and as such, the successfulcandidate will be instrumental in developing and evolving the service as theyprogress.

Our PCN is a dynamic, friendly network of like-mindedpractices, striving to provide effective, individualised care to over 37,000patients in the East Durham area. The successful candidate will benefit fromthe support of the wider PCN team and have expertise in carrying out long-termcondition reviews (including frailty) to patients within our aligned care homesand in the community for those who are housebound. You will be expected toutilise your skills in clinical assessment and diagnostics to make decisionsaround patient treatment plans. The successful candidate must be competent inall areas of chronic disease management.
