rimary Care Practice Nurse
to helpour Nursing team.

The vacancy ispart-time, around 20-25 hours per week over 3 days, although we do have someflexibility in total hours and working days. Please state your preferred hoursand working pattern in your application.

Main dutiesof the job

Ideally, you will have previousexperience of primary care nursing with the usual treatment room work,injections, smears, dressings, and chronic disease management.Pleasedetail your primary care experience in your application.

You must beprofessional, courteous, and efficient with excellent people handling skills,enthusiasm and the ability to work under pressure.

Main duties of the job

Main Nursingduties

• Providenursing care to patients as required in accordance with clinical basedevidence, NICE and local requirements
• Appraisewounds and other minor injuries, administer dressings; refer to a practicedoctor or the hospital as appropriate
• Takeswabs, cervical smears
• Provideimmunisations and injections to children and adults
• Assistwith Chronic Disease Management using the principles of MECC to maximiseencounters and minimise patient visits
• Supportthe Nursing team in dealing with clinical emergencies
• Monitorthe blood pressure of patients and consider indications for referral back tothe doctor
• Assistwith ECGs and report the findings to the doctors
• Assist thephlebotomists with venepuncture
• Insertring pessaries as directed by the doctor
• Assistwith INR clinics, including patient dosing using INRstar when a suitable HealthCare Assistant is not available

Chronic Disease Management

• Helpmaintain chronic disease registers
• Identify,manage and support patients as risk of developing long-term conditions,preventing adverse effects on the patients health
• Coronaryheart disease (CHD)
• Hypertension
• Smokingcessation
• Diabetes
• Asthma
• Chronicobstructive pulmonary disorder (COPD)
• Managementof other "Best Practice" patient registers

• Assist doctorsduring their surgeries e.g., chaperone, urgent electrocardiograph (ECG) etc.
• Beresponsible for maintaining a high standard of hygiene and cleanliness in thenursing area and of the clinical equipment used
• Monitorand maintain adequate supplies of drugs, clinical items and equipment
• Monitorand maintain emergency drugs bag
• Appropriatelyrecording all treatment undertaken in the patients record on the computersystem
• Ensuretemplates and read codes are used effectively
• Ensuremaximisation of QOF, Enhanced Services, IIF and other funding streams
• Obtain andmake use of health promotion material in clinics and for display purposes
• Implementnew and revised nursing procedures when necessary
• Attendmeetings, courses and update days as required
• Userelevant websites for the purposes of supporting the nursing role and givingadvice to patients

Managerial

• To workwith the doctors and Practice Manager to achieve the best possible service topatients
• Ensurethat the relevant requirements of the GMS Contract are implemented within thenursing department.
• Monitor,record usage and stock-take vaccines
• Discuss,implement and audit new nursing procedures
• Participatein audit and inspection as required by the GMS Contract, including cervicalcytology
• Assist thedoctors in the maintenance of relevant and accurate clinical histories
• Assist inoverseeing/supervision of Health Care Assistants
• Liaisewith external services / agencies to ensure the patient is supportedappropriately (vulnerable patients etc.)

Communication

• Teamplayer and attention to detail skills are essential
• Help andadvise reception/PLT, and other non-clinical, staff on clinical matters
• Advisepatients on the telephone
• Liaisewith the other members of the primary care teams and primary care networks
