Job summary - Working closely with the Acute Medicine Unit and other wards to provide assessment of patients with suspected ACS and facilitate their pathway from diagnosis to treatment.
- Liaising directly with the Interventional Consultant to provide a fast-track service ensuring that appropriate ACS patients benefit from early diagnosis and treatment.
- Identify and manage any barriers to their progress to treatment Arranging outpatient investigations as necessary and following up results in ACS virtual clinic.
- Triage and assessment of patients referred to Rapid Access Chest Pain clinic (RACPC) and liaising with a Cardiology Consultant to formulate a plan for further investigation and follow up as necessary.
- Liaising with the South London Cardiovascular Network and referring centres to facilitate inter-hospital transfer (IHT) for patients requiring coronary angiography on a 'treat and return' basis.
Main duties of the jobExpertly assess inpatients with suspected ACS, and triage and assess patients inRACPC, and advise on further management, in conjunction with a cardiologyconsultant.
Demonstrate clinical decision-making skills in referring patients for furthertests and following up results or discharge to GP.
Demonstrate clinical skills in interpretation of clinical investigations within the context of coronary disease.
Liaise effectively with all members of the multi-professional healthcare team and the South London Cardiovascular network to achieve a co-ordinated specialist service.
