An exciting opportunity has come about to work directly with rough sleepers in Camden as a Clinical Nurse Specialist for Inclusion Health. This post will form part of the Health Outreach Project (HOP) which is made up of this post and a psychologist. The team uses a flexible, outreach-based approach with the aim of addressing urgent physical and mental health needs.

The post is hosted by UCLH and co-located at Camden's Routes off the Streets (RTS) outreach hub (provided by Change Grow Live) in central Camden. HOP and RTS work in an integrated manner with the goal of being better able to meet the needs of rough sleepers experiencing multiple disadvantage and break down barriers to access to other services.

We are looking for a motivated individual who can lead and work in a flexible manner to best meet the needs of the rough sleeping population.

This post is fixed term until 31st March 2025, with the potential for extension pending confirmation of funding.
