  • Undertake Clinical Nursing Practice at an advanced level to deliver holistic care to people accessing primary health care services.
  • Assess, diagnose and treat patients contacting primary health care services with a range of acute and chronic medical conditions.
  • Prescribe medicines and appliances as determined by patient need.
  • Directly admit patients to secondary care hospital in acute medical need. Refer patients to other medical specialities or to other members of the multidisciplinary team for assessment if deemed appropriate.
  • Interpret diagnostic tests and routine clinical procedures.
  • Advise and support others as needed.
  • Work within professional guidelines and codes of conduct, acknowledging own limitations.
  • Work within all relevant practice policies and procedural guidelines, e.g. infection control. Chaperoning, risk management.
  • Independent Nurse prescriber.
  • Contribute to the practice targets e.g. QOF, ReVivo.
  • Contribute to disease registers.
  • Contribute to vaccination campaigns.

We are looking for an ANP/ACP to join our nursing team. The role is part-time with hours to be negotiated.

To ensure the delivery of an effective triage service by acting as an autonomous practitioner working independently and in conjunction with other health care professionals to assess, diagnose, and treat the conditions of patients within primary care initiating direct referrals as appropriate.

Provide professional advice that is evidence and research based, to patients, carers and colleagues and to ensure the maintenance of clinical excellence.

Follow national and local clinical guidelines and governance principles.

To provide education and training to other staff and students as needed.

Working as an independent practitioner preferably within a primary care setting or similar. Management of patients with long term conditions.

Minimum 1 year + experience at a senior level.

At least 2 years primary and community nursing experience (or similar).

Nurse-led management of minor illness, minor ailments and injuries.

Nurse-led triage.

Compiling protocols and clinical guidelines.

Accountability of own role and other roles in a nurse-led service.

Clinical governance issues in primary care.
