We also host the Sheffield Sexual Health Hub. Through our PCN membership we work collaboratively across our network area employing Pharmacists, Pharmacy Technicians,a Primary Care Mental Health team, Health Coach as well as a Social Prescribing team.
We have a very supportive and enthusiastic Management team, excellent Patient Services and Administrative support staff and strive to provide a supportive work environment.We support the green agenda and believe in low carbon transport and personal health & well-being.
We hold weekly team meetings including MDT, SEAand educational sessions. We also hold more informal daily lunchtime meetings for all clinical staff.
Our team are allocated dedicated CPD sessions based on the number of sessions they work and this allows us to continue to be at the cutting edge of primary care.

We are looking to recruit a Salaried GP to complement our experienced and diverse team. This position is a 4 month fixed term contract September until December 2024. This is an exciting opportunity for an ambitious GP to join our friendly, progressive team based in a dynamic Medical Centre in Sheffield.We offer a chance to join a well-established training practice with a high performing team.

Our premises are modern, and purpose built. Our team consists of 4 Partners, 6 Salaried GP's and a well established Nursing team including Advanced Clinical Practitioners, Practice Nurses,First Contact Practitioner, Nurse Associate and Healthcare Assistants. We use SystmOne, Ardrens and Accurx total triage for demand management.
In accordance with the Practice timetable, as agreed, the post-holder will make him/her-self available to undertake a variety of duties including surgery consultations, telephone / video consultations and queries, visiting patients at home, checking and signing repeat prescriptions and dealing with queries, paperwork and correspondence in a timely fashion
Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation
Assessing the health care needs of patients with undifferentiated and undiagnosed problems
Screening patients for disease risk factors and early signs of illness
In consultation with patients and in line with current Practice disease management protocols, developing care plans for health
Providing counselling and health education
Admitting or discharging patients to and from the caseload and referring to other care providers as appropriate
Recording clear and contemporaneous consultation notes to agreed standards
Collecting data for audit purposes
Compiling and issuing computer-generated acute and repeat prescriptions (avoiding hand-written prescriptions whenever possible)
Prescribing in accordance with the Practice prescribing formulary (or generically) whenever this is clinically appropriate
In general the post-holder will be expected to undertake all the normal duties and responsibilities associated with a GP working within primary care.
