We are looking to recruit a compassionate and dynamic Advanced Clinical Practitioner to join the team at Appletree Medical Practice in Duffield.

This is a great opportunity for an ACP with diverse skills to support healthy ageing amongst the fail and elderly population to keep them at home for longer and reducing hospital admissions, whilst also working as a primary care ACP, managing minor illness and common GP surgery presentations alongside the wider clinical team.

Additionally, the post holder will proactively manage the health needs of care home residents through regular ward rounds and anticipatory care planning.

A Band 8a position will be offered to candidates who have already completed an MSc in Advanced Practice (or equivalent) and who can demonstrate contemporary experience working at an advanced level within a relevant clinical environment.

A Band 7 trainee position may be offered to an aspiring Advanced Clinical Practitioner who is committed to undertaking an apprenticeship training programme which includes an MSc and a portfolio to demonstrate capability in the workplace.

The practice prioritises education and development so is looking for a self-motivated, enthusiastic individual with a desire to learn.

You will apply advanced clinical expertise, levels of judgement, discretion and decision making to the provision of clinical care to older people across primary and community care settings.

You will exercise a high degree of autonomy through the application of advanced clinical skills in relation to the assessment, diagnosis, treatmentand management of patients, in particular the older person.

You will lead and monitor improvements to quality of care through clinical audit, evidence-based practice, clinical supervision, teaching and supporting professional colleagues and the provision of skilled professional leadership.

You will lead the implementation of relevant clinical guidelines into practice and contribute to the further development of the evidence base throughresearch and audit.
