Job summary

The ideal candidate shouldhave a good understanding of Primary Care. a knowledge of Chronic Disease Management, Immunisation and Cytology experience or bewilling to undertake any necessary training required for the role. They should have an understanding of Qof andbe committed to providing excellent care to our patient population.

Main duties of the job

To work as a team with the Partners, Practice Nurse and administrationstaff to provide skilled and effective care to the practice population

To deliver care in accordance with the agreed Practice protocols andobjectives

To assess, plan, deliver and evaluate care holistically to the practicepopulation

Delivery of Chronic Disease management

Clinical Support on the management and care of patients with Long TermConditions

Plan, implement and evaluate individual treatment plans, within theremit of your own role

We are a 3 Partner, 3 Salaried GP, 3 ANP and 3 PN team supported by 2 HCAs and 1 TNA and a full Management and Admin team. We are situated next door to Sunderland Hospital in a purpose built building. We work together to provide the best service possible to our 10,300 patients from across the city.
