Rehabilitation Support Worker - Band 3
South Wiltshire Locality
37.5hrs per week - Permanent
Would you like to work in a friendly multidisciplinary team providing community based rehabilitation and support to patients with stroke and neurological conditions

We are looking for a Rehabilitation Support Worker to join the integrated community neurology and stroke service for the South area in Wiltshire, full time, permanent for 37.5hrs per week. The successful candidate will be providing high quality rehabilitation and support to service users working under the supervision and direction of the neurotherapists. The caseload is complex and varied, with a range of physical, cognitive and visual impairments. The neuro clinicians and assistants work very closely together to provide individually tailored rehabilitation programmes to address patient identified goals. Flexibility, together with good organisational and prioritisation skills, are essential to this community-based role.
All staff are expected to work flexibly across our sites and areas according to service need.
We are looking for an enthusiastic and dedicated team member who is committed to providing a high standard of care and rehabilitation to patients and their families. Through regular training, support and clinical supervision we provide opportunities for staff to develop their knowledge and skills within this specialist setting.
Rehabilitation Assistants work alongside the multidisciplinary team made up of Physiotherapists, Occupational Therapists, Speech and
Language Therapists and Dieticians. The majority of your work will be spent working directly with patients, in their own homes, to assist with rehabilitation programmes prescribed by the clinicians, and delivering some aspects of patient education. In addition, the post holder will support the team with non-clinical and administrative duties.
Successful candidates will preferably have worked in community settings and have experience working with people with stroke or other neurological or complex conditions.
Considerable amounts of driving are required and the post holder must therefore have a valid UK licence and access to a car.
We support staff in achieving a good work/life balance and are happy to discuss flexible working.
All candidates will be expected to work in a flexible way particularly to facilitate discharges.
