This Band 5 Prescribing Support post will work in partnership with Dietetics, Medicines Management and Primary Care to support and implement appropriate prescribing of nutritional products. It’s an exciting role including a wide range of clinical areas, direct and virtual patient contact, service development and the opportunity for quality improvement and audit.
The department fully encourages continuing professional development with protected sessions in job plans we actively support staff to participate and share learning opportunities and experiences.
To support and review the provision of effective prescribing of nutritional products and health care, to the people of the Cwm Taf Morgannwg University Health Board catchment area. To have clinical responsibility for a varied caseload made up of adult patients on oral nutritional supplements and other nutritional products.
To work as part of a multidisciplinary team which will include pharmacists, prescribing advisors, pharmacy technicians, GPs and other colleagues and agencies within the community.
To work autonomously and work within professional policies, standards and conduct as laid down by the British Dietetic Association, the Health Care Professions Council and Cwm Taf Morgannwg University Health Board Dietetic Department.
- To undertake searches for patients who are prescribed nutritional products on the GP computer systems e.g. Vision, EMIS.
- To assess, plan, evaluate and implement Dietetic interventions in outpatient clinics and community care settings, including community visits.
- To implement aspects of the treatment plan with autonomy, taking sole responsibility for the patient’s safety when undertaking community work.
- To work in partnership with other members of the multidisciplinary team
- To maintain clinical records in accordance with legal requirements and professional standards.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
