Job summaryBank Advanced Clinical Practitioner - Primary Care - GP OOHs
We are seeking highly motivated, suitably experienced and enthusiastic practitioners to join our team to provide an enhanced and quality Out of Hours service on behalf of Harrogate and District Foundation Trust.
We would like to offer the opportunity for individuals with extended skills in primary care to apply for a 'bank role' providing the flexibility to apply for adhoc shifts based in the Harrogate Out of Hours. Evening shifts daily 18:00 - 23:00 and daytime shifts Weekends and bank holidays.
The successful applicants will be responsible for ensuring that the highest standard of clinical care and initiate clinical assessment including telephone triage and consultation, diagnosis, treatment, referral using a range of advanced skills and expert knowledge for patients presenting with undifferentiated and undiagnosed conditions.
Main duties of the job - Use in-depth knowledge and highly developed clinical skills to assess, plan, implement and evaluate individualised care for patients
- Undertake home visits and clinical assessments in the patient's home as appropriate
- Order and interpret appropriate investigationsand act on results as appropriate
- Prescribing of appropriate medications/treatments within scope of practice using evidence based medicine
- Work across service boundaries and in partnership with primary and secondary care clinicians and social services, co-ordinating care and promoting a multi-disciplinary approach
