The role of Healthcare Assistant is fundamental to the success of Community Nursing Teams.

As a Community Healthcare Assistant, you’ll help deliver care to people in their own homes and get to work with inspiring, skilled teams whilst you do it. The role of Healthcare Assistant is fundamental to the success of Community Nursing Teams. The post holder will be able to demonstrate the competencies required to perform at this level and role model the Trust’s core values as well as exhibit the behaviours required as part of those values.

  • You will visit a number of patients on a daily basis in their own homes.
  • You will provide high quality nursing care to a range of patients with varying healthcare needs.
  • Deliver clinical and therapeutic care to patients within agreed trust policies/ protocols, as documented in the band 3 competency assessment tool.
  • Contribute to the holistic assessment and management of patients care needs through initial data collection, monitoring of patient’s progress, feedback discussion reports and in care reviews with the Community Nursing Team.
