As part of community transformation we are changing the way we manage referrals. We are developing a referral and assessment hub and are looking to recruit B7 Senior Practitioners to manage referrals and assessments within the hub.

Referrals will be triaged by the senior practitioner they will then make a decision on the onward journey for that service user. Each senior practitioner will be linked in with one of our CMHT's and that will be agreed on appointment.

You will provide duty assessments for the team you are linked in with and will complete those assessments at that designated base or at a venue agreed with the service user and carer. There may also be times when that duty assessment will be completed on the ward if they are an inpatient at the time of referral.

This post is a full time position (37.5 hours per week) and the working hours will be 9am - 5pm 5 days per week . The CMHT is currently operational Monday to Friday 9am to 5pm so flexible working can be discussed. The post is based at Cromwell House The post holder will be expected to travel across the Borough to attend satellite clinics across Salford.

This post includes participation in the Bronze on call rota.

Key duties willinclude:

  • Triage all referrals coming into the referral and assessment hub as part of a team. Providing an onward journey if not appropriate for Community Services.
  • Completing assessments for those people requiring specialist mental health services under Community Mental Health Team.
  • Complete assessment for those inpatients who may require a secondary service upon discharge.
  • To provide line management where required.
  • To link in with your MDT and attend their weekly meeting.
  • To develop and maintain relationships and pathways with other services
  • To promote awareness of the role of the referral and assessment hub, within local service area/team, GP practices and the wider community.
  • Supporting with service management in conjunction with the team manager
  • Working with the Team Managers to support with policy and service improvement and development
  • Supporting service user, carer and stakeholder involvement
  • Promote a culture of learning, education and professional development
  • Championing quality assurance by establishing quality standards relating to the service user and carer experience, implementing Divisional Action Plans and undertaking complaint investigations
  • Adherer to the principles of safeguarding of children and vulnerable adults in line with Trust Policy and local reporting procedure and ensuring junior staff compliance with this.
