Join Our Team as a Dementia Clinical Nurse Specialist
Are you passionate about transforming dementia care and leading a dedicated team to make a real impact? We are looking for an inspiring Dementia Clinical Nurse Specialist to champion clinical excellence and provide leadership across our acute hospital site, working alongside our Lead Nurse for Dementia.
Lead and Inspire the Enhanced Care Team:
Take the reins of our Enhanced Care Team (ECT) - a dynamic group of Band 3 Healthcare Assistants committed to delivering personalised, enhanced care to patients with cognitive impairments. Your leadership will be key in ensuring that every patient receives the compassionate, tailored support they deserve. You'll manage referrals, assess patient needs, and strategically allocate team members to provide the best possible care.
Drive Service Improvement:Be at the forefront of our mission to meet and exceed the National Dementia Strategy and NICE Guidelines. You'll collaborate with a wide range of professionals and stakeholders, from healthcare teams to carers and community partners, to elevate the quality of dementia care across the trust.
Deliver Clinical Excellence:
As an expert in dementia and delirium care, you'll provide invaluable guidance and support to multidisciplinary teams, ensuring that our practices are both cutting-edge and compassionate. Your expertise will shape the future of dementia care within our hospital and beyond.
The Dementia Clinical Nurse Specialist will be the specialist nurse for dementia & delirium and provide clinical leadership on dementia and delirium, under the supervision of the lead nurse for dementia, across the acute hospital site.
The post holder will help support the trust achieve national standards as set out in the National Dementia Strategy and the NICE Guidelines for dementia and will lead on the management of the enhanced care team - a team of band 3 health care assistants who are responsible for providing direct care to patients with cognitive impairment who require more intensive level nursing care including direct supervision. The post holder will also be required to take the lead in screening referrals, reviewing those patients who are deemed to require enhanced care and allocating a member of the ECT as appropriate.
The post holder will work alongside the lead nurse to enable the delivery of evidenced based practice and to implement the outcome recommendations of the National Dementia Strategy, NICE guidance, National Dementia Audit and the Dementia Friendly Hospital Charter (PLACE).
The post holder will provide expert advice and support to professionals involved in the care of patients with dementia and delirium, contributing towards service improvement in these areas, namely by meeting the NICE Dementia Quality Standards
The role is currently hospital based but will develop significant links with existing community teams.
