Integrate community services with acute providers working horizontally in a matrix with local Alliance partners to deliver community based services across the interface of health and care with primary care playing a key role.
Commitment to
one service delivery model
supporting the delivery of community services at neighbourhood level through integrated neighbourhood teams.
Commitment to an
aligned budget approach
, ensuring the cash envelope of community services, BCF funded out of hospital services and other community commissioned services are aligned to deliver the best outcomes for the residents of South West London.
The successful postholder will:
develop and implement a robust programme management approach to deliver phase 1 of the community model of care and provide recommendations for phase 2.
Work with the South West London contracting team to secure integration of community based contracts in each of the six SWL Places. Lead the development of service specifications, quality requirements, productivity measures and defined outcomes to achieve a core offer of community services across SWL. Lead the assessment of current community contracts / providers using the criteria set out in the provider selection regime.
Work with the South West London Director of Finance to make transparent the cash envelope for community services across each of the six Places testing value for money and making recommendations for consideration. Lead the procurement process mitigating risks to the SWL ICB
With senior representation from key stakeholders across South West London the Programme Board, Community Model of Care has been established to provide strategic and operational direction to the development of community services across South West London.
The prime responsibility of the Programme Board, supported by the Programme Director, is to secure continued development and delivery of a core offer of community services across all South West London Places.
The Programme Board will ensure the development of consistent quality requirements and set out the core outcomes to be achieved for the residents of South West London.
The programme board will deliver the strategy of integration of community services with acute providers in each Place supported by an Alliance of providers delivering across the interface of health and care through integrated neighbourhood teams.
