The Lewisham Care Partnership is seeking a Patient Care Navigator to serve as the first point of contact for patients at their central call center and practice sites in Lewisham. This role requires delivering excellent customer service by scheduling appointments, providing relevant healthcare information, and effectively signposting patients to appropriate clinicians based on their needs. The Patient Care Navigator will manage incoming calls, process appointment requests and repeat prescriptions, and maintain patient records using the Emis Web clinical system.

In addition to these primary duties, the Patient Care Navigator will assist with various administrative tasks, including managing queries, performing data entry for patient registrations, and maintaining a tidy reception area. The role may require occasional participation in practice audits and covering staff absences, making it essential for candidates to be adaptable and efficient in a busy healthcare environment. The position offers part-time hours, with a maximum of 37.5 hours per week.
