An exciting opportunity has arisen for a Transfer of Care (ToC) hub manager to join the Integrated Discharge team. The post holder will lead and manage the ToC hub .
The hub provides a central administrative resource for all complex patient discharges across the trust liaising with system partners and other providers of services to adults. The aim of the hub is to ensure a smooth, safe and timely discharge of patients who do not meet the criteria to reside and to work alongside system partners to ensure continuous flow within the acute service.
The manager will be responsible for the operational day to day running of the hub, and undertake all required escalations to resolve as an integrated team In order to reduce hospital discharge delays enthusiasm, communication, adaptability and an innate ability to problem solve and work through escalations are key skills required in for this post.
You will need to be able to remain calm under pressure as this can at times be a demanding environment. We require an individual who shows organisational skills, with a positive attitude towards flexibility and change.
The service covers seven day working 8am to 6pm, some weekend working and bank holidays would be expected as would occasional cross site working to cover holidays and periods of sickness.
The ToC hub is an essential component to support management and detailed understanding of the Trust & wider health economy's capacity. You will effectively lead & manage the ToC Hub staff and the day to day service delivery, and provide a high-quality ToC Hub which meets the administrative & reporting requirements of the Trust and its system partners.
You will need to manage yourself and your workload in a way that ensures the continued support of others.
