Job summary

An exciting opportunity has arisen to join Camden and Islington NHS Foundation Trust in our multidisciplinary Home Treatment Team (HTT) for Services for Ageing and Mental Health. The HTT works intensively with people with dementia and people with acute metal illnesses and age related frailty. The service is looking for a Band 5 clinical practitioner who is passionate about working with older people experiencing a mental health crisis and those who have dementia.

The service has an intense interface with psychiatric liaison services and inpatient services, providing gatekeeping assessments authorising admissions as well as facilitating early discharges; making this an ideal opportunity for an enthusiastic practitioner who wants to work in a well-established team and influence how care is delivered for this client group.

The post holder will work as an independent practitioner but within a supportive team to deliver care that enables service users to remain in their own homes during a period of mental ill health. Training opportunities will also be provided to enhance your knowledge around dementia and age related frailty.

Main duties of the job

The post is open to B5 Registered Mental Health Nurses including newly qualified staff and non-registered clinicians who can demonstrate suitable experience in working with service users experiencing a mental health crisis. We are looking for skilled and dynamic clinicians with a passion for delivering outstanding patient care.

Successful candidates will work alongside the Band 6 clinicians in the team to deliver high quality psychosocial assessments and interventions with older adults.

As a clinical practitioner you will be supported in completing assessments with a more senior clinician, working within an MDT model to discuss complex cases. As a new member of the team, you will receive a thorough induction to the service, monthly clinical supervision and regular reflective practice sessions.

Part of the Clinical Practitioner's role is ensuring that service users are supported through the initial stage of a crisis with high quality trauma informed therapeutic care and delivering effective care and interventions.

Reporting to the Senior Clinician or designated supervisor the post-holder will undertake a range of necessary tasks, including taking and screening referrals, prioritising referrals, identifying appropriate action in processing referrals, carrying out assessments, developing and agreeing care plans with service users, implementing and evaluating interventions, and identifying and referring on to appropriate support services for service users ongoing care.
