Job summary

An exciting opportunity has become available to join the Endoscopy services at UHBW in the new Care Navigator position. Care Navigators play a crucial role in helping people to get the right endoscopy care at the right time, managing a wide range of needs.Effective navigation is a key element of delivering coordinated person centred care and support.

We are looking for an individual who is able to demonstrate good problem solving skills, build strong lasting relationships, effectively utilise resources and serve as a source of expert knowledge for people accessing our endoscopy services.

Care navigation requires people to exhibit excellent communication in verbal and written form with a wide range of people from different cultural and organisational backgrounds. The successful applicant will be able to assert ideas and opinions in a respectful, positive way, which advocates the needs and wishes of the patient. The care navigation is taking an approach which supports a persons choice and wishes, as far as possible, enabling them to be in control of their own care.

This role will be delivered Monday to Friday excluding bank holidays. A 37.5 hours pw position is based at the Bristol Royal Infirmary and an 18.75 hours pw position based at Weston General Hospital with potential to work across all sites in UHBW.

Main duties of the job

  • In conjunction with the lead clinician, medical team, and senior nurse specialists, develop knowledge, skill and experience of the specialty including pathology, medication, prognosis, self-management treatment options and signposting patients to support that is available in the community.
  • Guide and support, staff, and patients on complex patient care issues to ensure a seamless transition for the patients between wards or home, and the unit.
  • Improve access for complex patients and ensuring equity of waiting times.
  • Working alongside system partners to demand and capacity manage complex patients at a regional level.
  • Provide an enhanced level of communication for complex patients, so that they are fully informed of their procedure and arrangements for this, as well as educating the endoscopy team about the needs of the patient.
  • Optimising the patient pathways to avoid delays, ensuring that the patient has the correct information about their procedure and understands their pathway.
  • Including access to General Anaesthetic and Propofol procedures, working alongside elective surgery to co-ordinate endoscopic procedures.
