The role of Care Navigator at Cheshire and Wirral Partnership NHS Foundation Trust in Birkenhead involves supporting young people and their families awaiting mental health treatment, within the Children & Young People's Mental Health Service (CYPMHS). The Care Navigator will primarily work in the community, meeting with families, liaising with professionals, developing access plans for community resources, and ensuring the well-being of young people by raising concerns with team leads when necessary.
