Are you passionate about enhancing patient care and ensuring smooth hospital transitions? Leeds Teaching Hospitals NHS Trust is excited to invite dedicated individuals, particularly women with a caring spirit, to apply for the role of Band 3 Discharge Co-ordinator. You'll be at the heart of our trauma services team, supporting patients as they move from hospital to home, making a real difference in their recovery journey. In this full-time position, you’ll collaborate with clinical staff, patients, and families to coordinate timely discharges, ensuring everyone is informed and comfortable with the process.
