The Leeds Teaching Hospitals NHS Trust is seeking a Band 3 Discharge Co-ordinator for trauma-related services at the Leeds General Infirmary. The role aims to improve patient experience by ensuring safe and timely discharges for patients, necessitating excellent communication, problem-solving skills, and collaboration with the multidisciplinary team. The Discharge Co-ordinator will be responsible for planning discharges from the point of admission, engaging with patients and their families, and coordinating with clinical staff to implement discharge plans.

The position requires flexibility as it operates seven days a week from 8 am to 6 pm, with some weekend and bank holiday shifts expected. Key duties include managing patient flows, communicating effectively with all stakeholders, maintaining accurate records, and participating in multidisciplinary team meetings to facilitate smooth discharges. Candidates should demonstrate a commitment to patient care and team collaboration while adhering to health and safety policies and safeguarding procedures.
