This is an incredible opportunity to become a vital part of our West Wirral Adult Social Care Community team, where your passion for supporting others can truly shine. You’ll work 30 hours a week on a temporary 12-month contract, helping to deliver personalised care that empowers local residents to live their best lives.

As a Care Navigator, you’ll play a pivotal role in promoting choice, autonomy, and social inclusion while working within the legal framework of the Care Act 2014. Your experience in supporting adults and families will be invaluable as you assess needs and connect individuals to the right services.
