Job Summary
The LTHT Cancer Pathway is designed for adults, where there is GP suspicion of cancer. This role has been designed to assist in coordinating the care of patients from point of referral, through the diagnostic pathway to diagnosis. It will require the post holder will need to adapt to support the pathway as it continues to evolve over time.
The Pathway Manager will provide comprehensive support to the Surgical teams within AMS through the co-ordination of primary and tertiary referrals and supporting patients throughout their diagnostic pathway.
The post-holder act as a single point of contact for patients and their families and as a liaison point between the GP, and medical and Surgical teams.
Main duties of the job
The post-holder will work both directly and indirectly with patients and their carers using a range of IT systems to assist in coordinating the care of patients through their hospital journey.
The post holder will assist in the preparation of telephone triage clinics, co-ordination of patient results and deal with actions following triage.
The post holder will coordinate appropriate services to maximise the delivery of streamlined diagnostic pathways, thereby improving quality of care and experience for patients and their carers.
The post-holder will support patients to access additional services during or immediately after their diagnosis (including support information and advisory services).
The post-holder will work across management teams and organisational boundaries, whilst linking closely with the Cancer Centre and contribute to plans and developments.
The post holder will be responsible for the review and updating of the tracking spread sheet which informs the cancer pathway.

Information / Data Collection
The Post-holder Will:
Develop and maintain a database to actively manage and record patient level detail.
Collect and record data on behalf on the consultants within the team, which can be manipulated to allow the extraction of data for audit and research purposes.
Using data gathered identify, develop and implement, through collaborative working with the clinicians improvements to the patient pathway and service overall.
Collect and record patient information from Trust data recording systems (e.g. pathology, radiology, outpatients, bookings, PPM database etc to ensure the patients progress can be measured.
Tracking and Case Management
The Post-holder Will:
Work closely with consultants, specialist nurses, clerical and other staff to deliver a coordinated, consistent management of patient cancer pathways.
Monitor the patients progress on the pathway and check daily to ensure that escalation points are adhered to.
Utilise reports in the PPM Database to monitor patients and identify delays for action.
Ensure patients are booked into next events in line with national standards & Trust policies, communicating with key personnel to action fast-tracking etc where required.
Liaise with other departments and trusts to ensure that investigations, opinions and treatments are sought in a timely fashion.
Ensuring all new cancer patients are tracked and highlighted for discussion at MDT meetings, when results etc are available.
Ensuring MDT members know which patients are listed for MDT meeting discussion so they can prepare slides, results and x-rays etc.
Ensuring that case notes, diagnostic test results, x-rays, audio/visual equipment etc., are available for each patient.
Escalation of Actions
The Post-holder Will:
Monitor patient pathways against the agreed timescales in accordance with the specified pathways.
Ensure that all relevant information is provided to the staff responsible for delivery of the cancer pathway.
Escalate those patients whose progress is likely to fall outside the agreed timescales.
Health and Safety/Risk Management
All staff are responsible for working with their colleagues to maintain and improve the quality of services provided to our patients and other service users. This includes complying at all times with the Leeds Teaching Hospitals NHS Trust Policies, including Health and Safety policies, in particular by following agreed safe working procedures, and reporting incidents using the Trust Incident Reporting system.
Infection Control
The jobholder must comply at all times with the Leeds Teaching Hospitals NHS Trust Infection Control policies, in particular by practising Universal Infection Control Precautions. Hand hygiene must be performed before and after contact with patients and their environment.
