The role of "Clinical Pharmacy Technician CRTs and Intermediate Care" at Aneurin Bevan University Health Board in Pontypool involves supporting the development of a Pharmacy Technician-led service within the Community Resource Teams (CRTs) and rehabilitation wards across five boroughs in the Gwent Region. The successful applicant will work collaboratively with the Lead Pharmacist and existing team members to enhance pharmacy services for frail and vulnerable patients, including those receiving care at home, in virtual wards, and in community hospitals. Key responsibilities include participating in multidisciplinary team meetings, conducting home visits and ward rounds, and contributing to the integration and redesign of pharmacy services that ensure patient care continuity post-discharge.
