Please note that this vacancy may be withdrawn at any time should it be filled via the internal redeployment process Job Overview The CRT Pharmacy team was set up in 2019 providing services to Newport, Caerphilly and Torfaen. Due to the continued success of the service, further investment was secured at the end of 2023 to extend the service to all 5 boroughs. The comprehensive extension to funding endorsed the contribution of this Intermediate Care Pharmacy team. The new funding not only allowed a roll out of the service to all boroughs within the Gwent Region, it also led to integration and redesign of the Pharmacy service provided to the Community hospital rehabilitation wards of South Monmouthshire and Torfaen. This new Pharmacy Technician post has been created to further support development of a Pharmacy Technician led pharmacy service to our virtual wards and rehabilitation wards. The successful applicant will be required to work across the 5 boroughs to support the Pharmacy service to the CRTs and help develop the new Pharmacy service to the Rehabilitation wards, working closely with the Lead Pharmacist and the current band 6 Pharmacy Technicians, increasing resilience within this developing service. Main duties of the job The CRT Pharmacy team currently provide a Pharmacy service to Frailty Consultant led ‘hospital at home’ patients, Therapies/Social Care-led Reablement patients, Nurse-led home parenteral service patients (including IV antimicrobials) and Community Falls patients. We also take referrals from acute ward Pharmacy staff to ensure that frail/vulnerable patients are supported in taking medicines post discharge. The new funding allowed integration and redesign of the Pharmacy service provided to the community hospital rehabilitation wards of South Monmouthshire and Torfaen. Our role involves regular MDT meetings, home visits, virtual & actual ward rounds and data collection and analysis of contributions to patient care. With a unique role working across the primary/secondary care interface, you will build excellent relationships with community, practice based and hospital colleagues. The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply. Qualifications And/or Knowledge Essential criteria NVQ Level 3 Pharmacy Services/BTEC in Pharmaceutical Science or equivalent Evidence of Continuing Educational and Personal development Knowledge of GPhC Standards for Pharmacy Professionals An understanding of leadership and management An understanding of Community Resource Teams and the primary/secondary care interface A basic knowledge of the pharmaceutical needs of frail/elderly individuals Desirable criteria NVQ Assessor BTEC Level 4 Professional Diploma in Pharmacy Clinical Services (or working towards/ prepared to work towards this) Basic knowledge of the functions of the NHS An understanding of health and social care structures ACT qualification or willing to be trained as ACT Experience Essential criteria Relevant post qualification community or hospital experience Near patient Pharmacy services Previous supervisory experience Desirable criteria Previous experience in a management role Previous experience in a teaching role Previous experience working with frail/elderly individuals Previous experience with domiciliary visits Aptitude and Abilities Essential criteria Good inter-personal skills Organisational abilities Good verbal and written communications skills Ability to work under pressure and to tight deadlines Basic keyboard skills Desirable criteria Ability to speak Welsh
