The Lead Community Diabetes Nurse role at York and Scarborough Teaching Hospitals NHS Foundation Trust focuses on enhancing diabetes care within the community by collaborating with GP practice staff to facilitate recovery and improve patient outcomes. This position involves leading and managing the Community Diabetes specialist nursing team, providing education and support to healthcare professionals and patients, and ensuring effective communication throughout the care process. The nurse will hold a defined caseload primarily composed of practices with low annual diabetes review completion rates, and will be expected to perform non-medical prescribing.

In addition to clinical responsibilities, the role encompasses service development in primary care, collaboration with various healthcare specialists, and participation in the education and training of primary care staff. The Lead Community Diabetes Nurse will also contribute to research and auditing to improve service quality.
